Paula Woolsey
Paula Woolsey was the former partner of Eric Saward and contributed a draft of the story Attack of the Cybermen to Saward during his time as script editor on Doctor Who. Doctor Who continuity advisor Ian Levine has claimed that he developed the story and Saward wrote the script; neither of them wished to have an on-screen credit.
